Start CARE Intake
The CARE Intake is a welcoming first conversation — not a test, not a form, not a judgment. It helps DCDC understand your goals and connect you to the right program. Click below to open the intake form. It takes about 10–15 minutes.
The CARE Intake form starts a conversation. It does not decide DVR eligibility, SNAP eligibility, program acceptance or payment.

Agile Work Profiler™
The Agile Work Profiler is a free tool from The DeBruce Foundation. It helps you discover your Agilities — the work activities you enjoy and do well. DCDC explains Agilities in plain language before you begin. You answer independently. Staff may help with technology but must not choose your answers. After finishing, select Save and Send My Results.
The Agile Work Profiler opens in a new browser tab. Your results do not diagnose a disability or guarantee career fit.
Send Agilities Results
After completing the Agile Work Profiler, use this form to send your results to your DCDC coach. You will share your top three Agilities, one result that surprised you, and one real example of using a top Agility.
