The CARE Intake is a welcoming conversation — not a test, not a form, not a judgment. It helps us understand your goals, your strengths, and what might be in your way so we can connect you to the right program.
What Is the CARE Intake?
CARE stands for Connect, Assess, Route and Engage. It is a structured but friendly conversation between you and a DCDC team member.
During the intake, we listen. We ask about your goals, your current situation, and anything that might make it harder to move forward. Then we help you choose the program that fits best.
The CARE Intake is the starting point for all three DCDC programs — Bridge to Success, Learning to Stay Agilities, and SHAPED for Destiny Agilities.

What to Expect
Complete a short intake form
You will answer a few questions about your goals and background. This takes about 10–15 minutes.
Have a conversation with our team
A DCDC team member will review your answers and talk with you about what you are looking for.
Get matched to the right program
Together, you will choose the program and funding track that fits your goals right now.
Meet your coach and begin
You will be introduced to your coach and take your first step on the pathway.

Anyone referred by a DVR counselor, school, workforce center, or employer
